Assignment:

Redesign/rearrange previous design to make it cleaner, more professional and simple.

Things I Did:

Created two concepts. Second concept chosen as new design. Coded new design in XHTML/CSS. Merged previous form and all of its PHP to new design. Took old design code from table-based layout to CSS. Created and coded in two days.

NOTE: Top stock images of medical professionals were chosen by previous designer, and were requested to be kept in new version by client.
